Event Consent Form

SUMMARY OF EVENT

This South West Alternative Provision (SWAP) event involves a maximum 12 students participating in a supervised "Escape and Evasion" activity at Larkbeare Brake, a secure woodland environment that has been pre-checked for safety.

The activity will run for four hours during the day/night. After the event, students will be transported to the SWAP site where they will stay overnight in one large American military tent capable of sleeping up to 20 personnel.

The hunter force, made up of current serving soldiers, will be integrated into the event in a non-contact role and escorted by SWAP staff. All personnel involved will have access to radios for communication, and roaming supervisors will be in place throughout.

This current event is scheduled to take place on Thursday 10th July 2025.

CONSENT

I understand the risks associated as laid out and accept that no responsibility for accidents or injuries or loss of damage to personal property rests with the supervisory staff, unless proven to be caused by their negligence. I declare that to the best of my knowledge, the young person whom I have parental responsibility for is competent and medically fit to participate in the activities as part of the group.

I agree that medical treatment will be given if necessary and in case of emergency. I understand the information from this activity may be stored digitally. I agree that a similar activity may be substituted due to safety factors or weather conditions.

I understand that any medical conditions will be communicated to SWAP before the commencement of the event.
